  • What do I need to bring to my first visit?

    If your doctor, nurse practitioner, P.A., or dentist gave you a written referral to visit our physical therapists in Albany, NY, bring that paperwork with you as well as your insurance card. If you were injured at work or in a car accident, bring all the insurance company information with you such as the claim number, case manager name and phone number, and address for the bills to be forwarded to so that we can correctly bill the carrier for your treatments.
