Billing & Fees
As a service to our patients, we do all primary insurance
billing, as long as complete information is provided at the time the first visit is scheduled. Columbia PT will gladly verify each individual's PT benefits and provide you with a written summary of the benefits. If a patient is responsible for a percentage of
payment or a copayment, it is due the day of service. We accept Visa, Mastercard, Discover and American Express. For
cases covered by Worker's Compensation or No Fault insurance,
it is the patient's responsibility to supply us with all
the information necessary to process claims.
We are participating providers with CDPHP, Empire Plan,
United Health Care, Blue Shield, BCBS, MVP, Wellcare, Medicare,
Worker's Compensation, No Fault, Shared Health Network,
US Aetna Healthcare, Private Healthcare Systems, GHI and
many others.
